What to bring

Arrive 15 minutes early to check in and change if needed.

Insurance & ID
Photo ID and insurance card (if using insurance). We’ll verify whether you have out-of-network benefits.
Comfortable clothing
Wear loose-fitting shirt and shorts so we can assess movement. Bring orthotics, glasses, or an oral appliance if you use them.
Relevant documents
Any referrals, imaging reports, surgical protocols, and a list of medications, symptoms, and questions.
